Job Description: Out of Area Flow Practitioner Band 6 Out of Area Flow Team, Sceptre Point Preston PR5 6AW. ASAP!29/02/2024Mon-Fri 30 hrs a week The OAPs team have responsibility for having oversight of all Lancashire and South Cumbria residents who are placed in an out of area bed. The Trust has several contracts with Independent sector Mental Health Acute and PICU beds providers and the team has clinical oversight of the patients in these beds. The team works closely with the Bed Hub and OAPs providers, monitoring patients in out of area beds and support with safe and timely discharge. Job description available on request.
