Permanent ANPReports to: Operations Manager Hours: 37 hours a week flexible Salary: Dependant on experience Essential: - RGN Qualification- Current NMC Registration- Honours degree in advsnced nursing- Minor Illness- Chronic disease management- Independant Prescribing Qualification- Experience in Permanent ANPReports to: Operations Manager Hours: 37 hours a week flexible Salary: Dependant on experience Essential: - RGN Qualification- Current NMC Registration- Honours degree in advsnced nursing- Minor Illness- Chronic disease management- Independant Prescribing Qualification- Experience in triage- Experience of dealing with patients with undifferentiated and undaignosed conditions- Health-needs backssment- Compiling protcols and clinical guidelines- Evidence of professional development- Experience in Audit Duties & Responsbilities- Providing backssment, screening treatment services and health education advice- backss, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. - backss, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with a minor illness. - Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. - Undertake care for patients in their homes as required by the Partners. - Provide regular care to patients in residential and nursing homes, ensuring all patients have up to date care plans. - Work with vulnerable patients to help reduce the need for unplanned hospital admissions ensuring all patients have up to date care plans in place. - Provide nursing treatment to patients in participation with general practitioners or independently within agreed protocols. - Provide general and specific health screening to the practice patients within the agreed protocols with referral.
